We already have a project tool. Does this replace it?

No, and don't make it. Keep shipping work where you ship it. What Hands adds is the staff doing the work between the work, and a record of what they did — the pipeline, the activity, the decisions waiting on you. If your project tool is where files live, a Hand uses it as a tool like anyone else would.

Can a client's Hand see another client's work?

No. Each client can be its own company in the app, with its own projects, files, memory and connected accounts. A Hand in one cannot list the staff of another, read its transcripts or reach its Gmail — that's enforced in the database, not the interface.
